Reporting a Medication Error

Pharmacy mix-ups occurring between two medications

24795b9d45550d61cc9f6bee7a09a629 MSeveral mix-ups between the medications INVEGA (paliperidone), which is used to treat schizophrenia, and INTUNIV (guanFACINE), used to treat attention deficit hyperactivity disorder (ADHD) have been reported to us.

In all cases the pharmacy mistakenly dispensed Invega 3 mg instead of Intuniv 3 mg. Each drug shares similar dosage strengths (3 mg), dosage forms, and dosing frequency, and both begin with “In” which increase the risk of confusion. Also, these products may be stored near one another on pharmacy shelves.

If you or a family member is receiving either of these two medications always look carefully to make sure you have the correct medicine when picking up your prescription at the pharmacy.
