It is important to keep track of all the medicines you take. This includes prescription and over-the-counter medicines, herbals, and supplements. The list should include both the brand name (if applicable) and generic name of the medicine, why you are taking it, the dosage, and how often you take it. It is good to always keep this list with you—in your wallet or purse—in case of an emergency. Review the list with your doctor at each appointment. Also, share this list with your pharmacist and a friend or family member.
The Agency for Healthcare Research and Quality (AHRQ) has a new resource that will help you keep track of all your medicines. “My Medicines List” is available for FREE in both English and Spanish. For a step-by-step guide on creating your medicine list using this resource, please visit: www.ismp.org/ext/1074.
