The Wall Street Journal recently published an article by patient advocate Laura Landro that discusses a new medication error alert system. The effort will initially be operated by the Institute for Safe Medication Practices (ISMP), which operates ConsumerMedSafety.org, and the American Society of Health-System Pharmacists (ASHP).

The system will alert doctors, nurses, pharmacists and the public about very serious medication errors and will offer guidance on how to prevent similar mistakes. Up to 26 other health care organizations are expected to join the network shortly.

Hind comes to ISMP from King Abdulaziz Medical City (KAMC) - National Guards Health Affairs, Saudi Arabia where she worked as a clinical pharmacist after she graduated from the University of Maryland School of Pharmacy, Baltimore, MD in 2004. KAMC is a 950-bed, acute-care, tertiary, teaching institution with a level-1 trauma center. Hind also has a B.Sc. degree in pharmacy and is a Board Certified Pharmacotherapy Specialist. She is a fellow of the American College of Chest Physicians (FCCP). Hind served in the Medication Safety Program (MSP) at KAMC, where she helped implement many of the ISMP/JCAHO recommendations there.

ISMP welcomes Rebecca (Becky) Lamis, PharmD, as the ISMP Safe Medication Management Fellow for 2009-2010. Becky received her PharmD at the University Of Iowa College Of Pharmacy in Iowa City, IA, and completed a Pharmacy Practice Residency at Wesley Medical Center in Wichita, KS. Becky’s fellowship is being supported through a grant from the Cardinal Health Foundation.

ISMP also welcomes Alice Tu, PharmD, who is the FDA-ISMP Safe Medication Management Fellow. This is a joint FDA-ISMP fellowship during which Alice will work 6 months at ISMP and 6 months at FDA in the Center for Drug Evaluation and Research, Division of Medication Error Prevention and Analysis. Alice received her PharmD from the University Of North Carolina Eshelman School Of Pharmacy in Chapel Hill, NC, and completed a Managed Care Pharmacy Residency at Medco Health Solutions in Franklin Lakes, NJ.
